Financial Aid Forms

Financial Aid Forms

Financial Aid requires submitting many forms. Only complete the forms that were requested from you by the Financial Aid Office, submitting additional documents that have not been requested by the Financial Aid Office may delay the processing of your financial aid awards. The Financial Aid Office mails sends emails to applicants notifying them of what documents have been requested. Please refer to either of those communications or visit your Financial Aid Self-Service Page. The sooner you complete and return the required forms, the sooner we will be able to determine your eligibility for financial aid.

HOW TO USE THE IRS DATA RETRIEVAL TOOL AND REQUEST TAX TRANSCRIPTS FROM THE IRS.

If you are unable or ineligible to use the Data Retrieval Tool then you can request a tax transcript from the IRS in one of three ways.

  2. Call 1-800-908-9946

It takes up to two weeks for IRS income information to be available for electronic IRS tax return filers, and up to eight weeks for paper tax return filers. If you are married, and separate 2015 tax returns were filed, 2015 IRS tax return transcripts must be submitted for both you and your spouse.
